In this article, we summarize a number of recent major incidents and review important aspects of risk management and protection through relevant insurance.
Deadly Collision on the Gempol Toll Road! AKAS Bus Rammed By Wingbox Truck, Driver Crashed to Death
A tragic accident occurred on the Gempol KM 796 toll road section towards Probolinggo on Monday morning (14/4/2025). An AKAS bus with plate number N 7075 UR hit a wingbox truck loaded with packaged drinks from behind, causing the bus driver to die on the spot.
According to Aiptu Hari, a member of the East Java 3 Highway Patrol (PJR), this terrible incident occurred at around 06.00 WIB. The bus traveling from the west was allegedly traveling at high speed and hit a wingbox truck number B 9055 UEV which was on its way from Jakarta to Denpasar, Bali.
“The wingbox truck was carrying a load of packaged drinks belonging to Unilever. When it arrived at KM 796-200, from the same direction, the bus hit it from behind,” said Aiptu Hari to Radio Suara Surabaya.
The hard impact caused both vehicles to bounce and cross the road guardrail, changing lanes towards Surabaya. The two vehicles finally stopped in the chevron area of the opposite lane, causing traffic jams due to spilled cargo.
What was most heartbreaking was that the AKAS bus driver died after being severely trapped by the front of the bus which hit the back of the truck. The process of evacuating the victim was dramatic because the position of the victim’s body was difficult to reach.
It is suspected that the accident was caused by the bus driver being sleepy and failing to maintain a safe distance. It is known that the bus only carried five people, including the crew. Apart from driver negligence, another factor that aggravated the impact of the accident was the road guardrail which had not been installed properly at the location.
Officers are still trying to evacuate the two vehicles which are closely related to each other and clean up the remaining scattered cargo so that traffic can flow smoothly again.

The fire incident that shocked Surabaya residents occurred early this morning at a popular meat grill restaurant, Rest Fork, whose address is Jalan Tunjungan No.38, Genteng Village. This terrible incident took place at around 00.51 WIB, Wednesday (16/4/2025).
According to Amin Akbar, an officer from the Surabaya Fire and Rescue Service (PMK), the first report came from a resident named Gilang. It didn’t take long for two fire brigades – one combat fire unit from Rayon 1 Pasar Turi and one rescue team unit – to immediately rush to the location.
“The team arrived at 00.56 WIB, the fire was extinguished in just three minutes, and the wetting process was completed at around 01.12 WIB,” said Amin.
The cause of the fire is thought to have come from oil droplets leaking onto the grill. The fire source is located in a 3×2 meter grilling area in a 30×15 meter restaurant building. Blocked oil channels trigger oil leaks to hot spots, causing sparks.
As a result of this incident, an employee named M. Rifky (21) suffered serious burns to his left arm. He immediately received treatment from PMI and was rushed to William Booth Hospital.
Luckily, thanks to the quick response from PMK, BPBD, Satpol PP, and PMI, the fire did not spread to other parts of the restaurant and was controlled quickly.
Although material losses have not been disclosed, this incident became important warning about restaurant kitchen safety, especially those using intensive roasting systems.

Plane Crashes in New York! Holiday Turns Into a Nightmare, 6 People Die
An air crash rocked eastern New York on Saturday afternoon (13/4/2025) when a small Mitsubishi MU2B40 plane crashed in the Copake area, Columbia County. This tragedy claimed the lives of six people, all of whom were members of one family on a holiday trip.
According to US aviation and security authorities, the plane crashed only about 16 kilometers from Columbia County Airport, the final destination of their flight. The plane made an approach to the runway, but failed and asked for permission to try again—unfortunately, that second chance ended in death.
“The pilot reported a failed approach and requested a re-vector, but then contact was lost,” said Todd Inman of the National Transportation Safety Board (NTSB) in a press conference held shortly after the incident.
The dead included pilot Michael Groff, a renowned neurosurgeon, his wife Joy Saini, also a doctor, as well as their two children and their respective partners. The six were on their way to a family celebration when disaster struck.
Bad weather is thought to be one of the main causes of accidents. The drastically reduced visibility made landing even more difficult, even though the pilot was known to be very experienced and the plane had recently received a cockpit upgrade.
“The plane was found crushed and embedded in the ground due to the heavy impact,” explained Inman.

Burning Plane Crashes Near Toll Road! Three Dead, Car Crashed by Crashing Tree in Florida
A shocking tragedy occurred in Boca Raton, South Florida, when a small Cessna 310 plane crashed and caught fire just moments after taking off from Boca Raton Airport on Friday morning (around 10:20 local time). The plane was en route to Tallahassee, North Florida.
In this horrific incident, three people reportedly died on the spot. In addition to the victims from the plane, a driver who was passing on Interstate 95, where the plane crashed, was injured after his car hit a tree while trying to avoid falling pieces of plane debris. Luckily, the injuries he suffered were not life-threatening.
Firefighters were immediately dispatched to the location to extinguish the fire and secure the area. Several main roads around the scene were immediately closed to the public.
The Federal Aviation Administration (FAA) together with the National Transportation Safety Board (NTSB) are currently conducting a thorough investigation into the cause of the accident. The NTSB is leading the investigation, including analyzing flight data and the pilot’s last communications.
Boca Raton Mayor Scott Singer expressed his concern and asked the public to give space to the victim’s family. “We are deeply saddened. This is a huge tragedy in our community,” he said.
The crash adds to a long list of air incidents in the United States in recent months, and again raises big questions about the safety of civil aviation, especially small planes.
