The past week’s numerous accidents and fires have once again drawn public attention and served as a reminder of the importance of risk management and insurance protection. From the multiple accidents on the Cipularang Toll Road, the cigarette warehouse fire in Pamekasan, and the passenger ship incident in North Kalimantan, each incident resulted in losses that were not only material but also had a far-reaching impact on business continuity and public safety. In the business world, any risk that is not properly anticipated can lead to significant losses. Therefore, companies need to increasingly prioritize risk management strategies and insurance protection as an integral part of business continuity.

Horrific Collision on the Cipularang Toll Road! Two Buses and a Jasa Marga Tow Truck Involved

A pile-up involving two buses and a Jasa Marga tow truck occurred at kilometer 99 of the Cipularang Toll Road in West Java on Sunday afternoon (September 21, 2025). The tow truck driver suffered serious injuries, while four passengers on the tour bus were rushed to a hospital in Padalarang.

The incident involved a Primajasa bus with license plate number B 7075 PW, a tour bus with license plate number B 7076 SGA, and a Jasa Marga tow truck with license plate number D 8534 QF. The tour bus was carrying a group of women on a pilgrimage to Bandung.

Chronology of the Accident

The accident began as a tour bus was traveling from Jakarta to Bandung at approximately 80 km/h. At the same time, a Primajasa bus coming from the opposite direction (Bandung-Jakarta) was unable to brake when a tow truck attempted to turn around at KM 99.

The Primajasa bus struck the crane from behind, pushing the vehicle into the Bandung-bound lane. As a result, the crane, which had lost control, collided directly with a tour bus traveling from Jakarta.

“The Primajasa bus driver failed to anticipate the situation and hit the tow truck. The tow truck then slid onto the Bandung lane and collided with the tour bus,” explained Ginanjar, the tour bus driver.

Victims & Handling

Fortunately, there were no fatalities in the incident. However, the Jasa Marga tow truck driver suffered serious injuries, while four passengers on the tour bus suffered minor injuries. All victims were evacuated to Padalarang Hospital in West Bandung for treatment.

An unfortunate incident befell Sutrisno, a water channel project worker in the West Gayungsari area, Gayungan, on September 16, 2025. This accident has once again opened the public’s eyes to the importance of implementing Occupational Safety and Health (K3) in every construction project.

According to Widodo, a former K3 officer on the Suramadu Bridge project, this case should be a valuable lesson.

“Worker safety must be a top priority. Unfortunately, many still believe that implementing K3 will waste money and slow down project progress,” he explained on Tuesday (September 23, 2025).

While implementing strict OHS standards does require additional costs and longer processing times, compared to the risk of workplace accidents that can result in significant losses—both material and human life—the investment in OHS is far less costly.

A Wings Box Truck Crashes into a Vehicle and a House in Pasuruan, Killing the Driver

A fatal accident occurred on the Surabaya-Malang Highway, precisely in Purwosari Village, Purwosari District, Pasuruan Regency, on Monday (22/9/2025).

A wing box trailer truck with license plate number N 9458 UG, carrying paper rolls, was traveling from Malang to Surabaya. While traveling downhill, the truck allegedly suffered a tire blowout, causing the driver to lose control.

As a result, the truckcrashTwo parked vehicles, a residential terrace, and several electricity poles were involved. The incident resulted in the truck driver, Komaru Sabar, dying at the scene after being trapped under the truck’s hull.

Additionally, a pickup truck carrying cooking oil was hit, spilling its cargo onto the road. A Suzuki Ertiga was also grazed from behind. The pickup driver, Makruf, suffered minor injuries and is receiving treatment at the Purwosari Community Health Center.

Police who arrived at the location immediately evacuated the victims and normalized traffic flow using a contra flow system and diverted four-wheeled vehicles to the Purwodadi Toll Road to reduce congestion.

Manohara Borobudur Hotel Engulfed in Fire, Suspected Electrical Short Circuit

An early morning fire engulfed the Borobudur Cultural Center (BCC), also known as the Manohara Hotel, in the Borobudur Temple complex in Magelang Regency, on Saturday (September 20, 2025). Preliminary suspicions suggest the blaze was caused by an electrical short circuit.

Head of the Magelang Regency Fire Department, EdyPriyanta, said his team received a report of the fire at 12:30 a.m. WIB. Eleven fire trucks were deployed from various nearby posts due to the location’s proximity to headquarters.

The building’s largely wooden construction caused the fire to spread quickly. However, within about 30 minutes, the blaze was contained and then cooled down.

The fire affected one of the buildings used as a meeting room. The Director of Borobudur Tourism Park (TWB), Mardijono Nugroho, confirmed that the incident was handled well and resulted in no casualties. However, details of the losses and the details of the burned buildings are still being compiled.

PT SHM Jaya’s cigarette warehouse was completely destroyed by fire, causing losses of Rp 35 billion.

A cigarette warehouse belonging to PT SHM Jaya, located in Blumbungan Village, Larangan District, Pamekasan Regency, East Java, was engulfed in flames on Thursday night (September 18, 2025). The fire was reported at around 8:15 PM WIB, and two fire trucks arrived at the scene 10 minutes later. However, the flames had already spread, destroying the warehouse’s contents.

Of the four cigarette production machines, three suffered minor damage, while one was severely damaged. Raw materials, including tobacco, and other items in the warehouse could not be salvaged.

Herwanto, Head of the Pamekasan Public Order Agency (Satpol PP) and Fire Department Fire Department Fire Department Firefighters, explained that the fire was suspected to have been triggered by an electrical short circuit, which then spread through the wiring and ignited production machinery. The roof and walls of the warehouse were also burned.

After nearly 3.5 hours of fighting, the fire was finally extinguished at around 11:50 PM WIB. Based on a preliminary investigation with the police, the estimated damage from the fire reached Rp 35 billion.

A passenger ship in North Kalimantan leaks after colliding with driftwood.

A sea accident befell the Malinau Express 6 passenger ship which was sailing from Malinau Regency to Tarakan City, North Kalimantan, on Monday (22/9/2025) afternoon.

The ship, carrying 35 passengers, sank after its hull leaked, allegedly after hitting a drifting log in the Sesayap River. According to reports, the ship hit the log twice before developing a serious leak and began taking on water, causing panic among passengers.

The Head of the North Kalimantan Transportation Agency, Idham Chalid, confirmed the incident. Fortunately, all passengers were rescued by another passing ship, resulting in no casualties.

“Thank God, all the passengers are safe,” said Idham with relief.

Following the incident, the North Kalimantan Transportation Agency urged all ship operators to tighten safety standards, particularly ensuring the availability of life jackets according to the number of passengers.

This incident adds to a growing list of water transportation accidents in North Kalimantan, which still relies heavily on river routes. Natural factors such as drifting logs and strong currents often pose challenges for skippers.

The wreck of the Malinau Express 6 remains at the site, awaiting recovery. Authorities will conduct an investigation to determine the exact cause and to tighten maritime safety standards.

A carpet warehouse in Tanjung Priok was destroyed by fire, causing losses of Rp2.5 billion.

A massive fire engulfed a carpet warehouse belonging to PT Ideal Carpet Indonesia on Jalan Agung Karya 5, Papanggo Subdistrict, Tanjung Priok, North Jakarta, on Tuesday morning (September 23, 2025). The fire started at around 6:20 a.m. WIB and was fully extinguished by 10:00 a.m. WIB.

The disaster caused material losses estimated at Rp 2.5 billion. Gatot Sulaeman, Head of Operations for the North Jakarta Fire and Rescue Sub-dept., explained that the fire is suspected to have been caused by an electrical short circuit. According to security personnel, sparks were seen in the electrical circuit breakers, which then ignited flammable materials, causing the fire to spread rapidly.

To tame the fire, the Gulkarmat Sub-dept. deployed 19 fire trucks and 106 personnel. Thanks to the hard work of the officers, the blaze was finally brought under control after nearly four hours of extinguishing efforts.
