Risk Accident Review
Fishing vessel fires in the open sea are a low-frequency but high-severity risk. Common potential causes include electrical short circuits, fuel leaks, engine overheating, or negligence in safety procedures. In addition to the total physical loss of the vessel (constructive total loss), there is a risk of loss of life, legal liability claims, operational disruption, and loss of revenue. At-sea extinguishing operations are limited by resources and response time. Without proper mitigation and insurance protection, vessel owners can face significant financial pressures, including asset replacement, crew compensation, and potential third-party claims.

Welding Sparks Lead to Disaster! A Gran Max Burns Down, and Two Shophouses Also Burn in Malang
A Daihatsu Gran Max caught fire in front of a welding workshop on Jalan Tenaga Utara, Purwantoro Village, Blimbing District, Malang City, on Tuesday (February 24, 2026) at around 2:30 p.m. WIB. The fire is believed to have originated from welding sparks during the vehicle repair process.
According to eyewitnesses, the fire initially appeared small before growing and engulfing the entire car. Several explosions were heard, estimated to be up to four, discouraging nearby residents from approaching to extinguish the flames.
The flames then spread to two shophouses used as a workshop and shop. Residents reported that one of the occupants managed to escape through the back door.
Malang City Fire Department officials suspect welding sparks ignited flammable materials, including vehicle tires and the interior of the workshop. When officers arrived, the car and two buildings were already severely burned. Two fire trucks were deployed to control the blaze.
In addition to the Grandmax car, some of the workshop’s contents were damaged, including a motorcycle. Two leaking LPG cylinders were recovered by officers to prevent further explosions. There were no fatalities or injuries in the incident.

Risk Accident Review
Fires resulting from welding activities fall into the hot work risk category, which carries the potential for high losses without strict safety procedures. Sparks can easily ignite flammable materials such as fuel, tires, oil, or LPG. The risk extends beyond the vehicle being repaired to buildings, third-party assets, and potential liability claims. In addition to physical losses, workshop owners face potential compensation claims for damage to customer vehicles. Without proper insurance coverage, a single fire incident can erode business capital and significantly disrupt operational continuity.
Types of Insurance Needed
- Property All Risks (PAR)
Covers damage to workshop and shop buildings due to fire and other risks. - Motor Vehicle Insurance
Protect customer vehicles or private vehicles from the risk of fire and total damage. - Public Liability Insurance
Covers third party claims resulting from workshop operational activities that cause losses. - Business Interruption Insurance
Cover potential loss of income due to workshop operations being halted following a fire.
In a technically based business like a repair shop, the biggest risk is often not the work itself, but the potential for cascading losses from one small spark.
As an insurance broker and consultant, it is important to conduct a risk assessment of hot work activities and ensure that the policy structure is in accordance with the business risk exposure.
Workplace Accident! Truck Hits Forklift, Driver Killed in Pelindo Banten Area
A work accident occurred in the area of the Indonesian Port (Pelindo) Banten, Ciwandan District, Cilegon City, on Monday (24/2/2026) at around 10.00 WIB. The incident killed a truck driver with the initials A (27), a resident of Pandeglang Regency, Banten.

Risk Accident Review
Public transportation accidents due to driver fatigue constitute a serious operational risk in the mass transit industry. Suboptimal fatigue management can lead to loss of concentration and even fatal accidents. Risks include not only vehicle damage but also mass passenger injuries, medical claims, lawsuits, and potential damage to the operator’s reputation. In these cases, the potential for bodily injury claims is significant because of the large number of victims involved. Public transportation operators are required to have a strict safety management system, including driver work schedules and health monitoring. Without comprehensive insurance protection, exposure to claims can burden a company’s cash flow and financial stability.
Types of Insurance Needed
- Motor Vehicle Insurance
Covers physical damage to the bus due to collision. - Public Liability Insurance
Protects operators from third party claims resulting from transportation service operations. - Employers Liability / Workmen Compensation
Covers the risk to the driver and crew if they are injured while on duty. - Directors & Officers (D&O) Liability Insurance (for major operators)
Protecting management from potential lawsuits related to negligence in safety system oversight.
In the public transportation industry, a single moment of fatigue can escalate into a mass incident with major financial impact.
As an insurance broker and consultant, it is important to ensure that the protection structure covers vehicle risks, legal liability for passengers, and comprehensive operational risk management.
SirenThe barrier has already sounded, but the barrier hasn’t closed yet! A container truck was hit by an airport train in Poris, Tangerang. Here’s the chronology.
An accident between a container truck and an airport train on the Manggarai–Soekarno Hatta route occurred at the Poris Station level crossing, Batuceper, Tangerang City, Friday (20/2/2026) at around 06.05 WIB during heavy rain.
Tangerang City Metro Police Chief Raden Muhammad Jauhari explained that, based on initial information, officers had sounded the warning siren. However, the automatic barrier did not immediately close. The truck, traveling from Cipondoh toward Jalan Daan Mogot, continued to pass through until it entered the railroad tracks.
As the barrier began to close, the container was already on the tracks. The truck’s head section managed to clear the track, but the container’s body remained behind. A passing train then struck and dragged the container for about 100 meters.
There were no fatalities in the incident. The driver and conductor were unharmed, but material damage was significant. Two electric poles operating on the KRL were reportedly knocked down, causing the train to lie across the tracks, blocking access to Jalan Raya Maulana Hasanuddin. The evacuation involved joint personnel and KAI technical personnel, and traffic engineering was implemented to ease the congestion.
Risk Accident Review
Accidents at level crossings are a transportation risk with the potential for catastrophic losses. Weather factors, human error, and potential delays in automated systems (sirens and barriers) create a dangerous combination. The impact extends beyond vehicles to damage to public infrastructure, disruption to train operations, third-party claims, and potential lawsuits. In this case, losses include damage to container trucks, train electrical facilities, disruption to public services, and potential business interruption. Without proper insurance coverage, both vehicle owners and transportation operators can face significant financial burdens due to multi-party compensation claims.
Types of Insurance Needed
- Commercial Vehicle Insurance
Covers physical damage to trucks due to collisions, including the risk of total loss. - Third Party Liability (TPL)
Covers third party losses, including damage to public facilities such as electricity poles and rail infrastructure. - Public Liability Insurance
Protecting the company from legal claims resulting from disruption of public operations or losses to other parties. - Employers Liability / Workmen Compensation
Provides protection for drivers and conductors in the event of injury or death while working. - Business Interruption Insurance (for transportation operators)
Cover potential loss of revenue due to operational disruptions.
Each of the incidents mentioned above has a different risk profile: marine risk, transportation risk, industrial fire risk, and even occupational accident risk. However, one thing remains in common: without proper risk management and insurance structure, the impact can significantly erode a business’s finances.
Insurance is not merely an administrative obligation, but a strategic risk mitigation instrument to maintain business continuity, protect assets, workforce, and company reputation.
As insurance brokers and consultants, our role is not only to provide policies, but also to ensure that every risk is identified, calculated, and optimally protected.
Because in the business world, the unexpected is not the question of “will it happen”, but “when will it happen and are you ready?”.
