Head of Blitar City Police Traffic Unit, AKP Andang Wastiyono, explained that the articulated truck was on its way to Kediri. When crossing the railroad tracks, the door latch at the scene of the incident was open.
“The doorstop guard was known to have fallen asleep so he didn’t close the gate when the train passed. “As a result, there was a serious collision between the train and the truck,” said AKP Andang on Tuesday (24/12/2024).
This incident started when the truck almost managed to pass the crossing. However, the trailer part was still on the tracks when the Kertanegara train came from the west at high speed. The officer guarding the gate had tried to close the gate when he realized the presence of the truck, but his efforts were too late.
Lack of Signs and Safety
AKP Andang also highlighted the lack of signs warning of trains passing at this location.
“There were not enough warning signs to alert the driver that the train was moving. “This is one of the causes of trucks getting stuck on the tracks,” he added.

Tragedy on the Pandaan-Malang Toll Road, Deadly Collision of Truck and Bus for School Group. Four People Killed
A tragic accident occurred at Kilometer (KM) 77+200 Pandaan-Malang Toll Road, East Java, on Monday afternoon, December 23 2024. This incident involving a bus and a truck claimed four lives. Head of Malang Resort Police (Polres), AKBP Putu Kholis Aryana, revealed that one of the victims who died was the bus driver, while the other three consisted of bus crew and passengers.
“Initially we found one dead victim. However, after the evacuation process with the team, the total death toll increased to four people, including the bus driver,” said AKBP Putu, quoted from Antara.
Victims and Treatment
The four victims who died were taken to the Regional General Hospital (RSUD) Dr. Saiful Anwar, Malang City. Meanwhile, the survivors were rushed to Lawang and Lawang Medica Regional Hospitals for treatment. Based on the passenger manifest, the bus with plate number S 7607 UW was carrying a group of students from SMP IT Darul Qur’an Mulia Putri, Bogor, West Java.
“According to the manifest data we received, there were 40 students in this group. Apart from that, there were crew, companions and drivers. We are still investigating the total number of passengers,” added AKBP Putu.
This group is known to have traveled from the Mount Bromo tourist area and planned to go to English Village in Kediri Regency. However, further information regarding the travel route has not been obtained, considering that many passengers are still in a traumatized condition.
Conditions and Causes of Accidents
The front of the bus suffered serious damage due to the collision. The wheelhouse appeared to be damaged, indicating the severity of the impact. Until now, police officers and PT Jasamarga Pandaan-Malang are still carrying out investigations to determine the cause of the accident. The traffic flow around the accident location was observed to be slow, because only one lane was open towards Malang.
The President Director of PT Jasamarga Pandaan-Malang, Netty Renova, apologized to road users for the traffic jam that occurred as a result of this incident. “We apologize for the inconvenience caused. We urge road users to always ensure their vehicles are in roadworthy condition and maintain their health while traveling,” said Netty in a written statement.
Handling the Impact of Accidents
As a mitigation measure, Netty advised road users to exit via the Purwodadi Toll Gate and re-enter the Pandaan-Malang Toll Road via the Lawang Toll Gate. He also reminded road users to contact Jasa Marga’s One Call Center service at number 14080 or the Travoy 4.4 application to get help and information about toll roads.
The accident, which occurred at around 15.40 WIB, involved a class 3 truck and a bus, with serious damage to both vehicles. Joint officers continue to work to deal with the impact of this incident and return toll road conditions to normal.

PT Yura Engineering employee dies when crushed by an excavator and hits his head
A heartbreaking work accident incident occurred in Bekasi City, West Java, on Friday (20/12/2024). A worker named HRF died after being hit by a heavy excavator at the project site located at Jalan Neman Jaya number 100, RT 05/RW 18, Pengasinan Village, Rawalumbu District.
“On Friday, December 20 2024 at 14.41 WIB, a work accident occurred which resulted in the victim’s death. This report has been received by Polda Metro Jaya,” said Head of Public Relations of Polda Metro Jaya Kombes Pol Ade Ary in a written statement, Sunday (22 /12/2024).
Chronology of Events
The incident started when seven PT Yura Engineering employees, including HRF, arrived at the project area at around 08.00 WIB. Workers were scheduled to move the amphibious excavator heavy equipment during the day.
At around 14.05 WIB, the process of moving heavy equipment began using a 25 ton capacity crane. However, misfortune cannot be avoided. At 14.41 WIB, the excavator that was being lifted rolled to the right.
“The excavator that rolled over hit the head of the victim who was in the lifting area at a distance of approximately two meters,” explained Ade Ary.
Rescue Efforts
Not long after the incident, the victim was immediately rushed to the East Bekasi Primaya Hospital at around 14.50 WIB. However, HRF’s life could not be saved due to the serious injuries he suffered.
Further Research
This work accident case is currently being handled by the Rawalumbu Police, Bekasi City. Officers are trying to uncover the cause of the excavator’s overturning, including evaluating work safety procedures at the project site.

Tragic Work Accident, Chip Officer Killed by Falling Logs in Berau
A tragic work accident incident occurred at the PT Bahtera Sejahtera log company located in Labanan Jaya, Teluk Bayur District, Berau Regency, East Kalimantan, on Tuesday (24/12/2024). This accident claimed the life of a Chip Officer named Agus Winarno (48), who died as a result of being hit by a landslide of wood.
Teluk Bayur Police Chief, Inspector Alimuddin, confirmed the incident and explained the chronology of the incident which took place in the morning at around 08.15 WITA.
Chronology of Events
According to information, the incident started when the heavy equipment operator, Helmi Arifin (69), was moving a pile of logs at the direction of Agus Winarno. During the moving process, the pile of wood suddenly collapsed.
“The pile of wood immediately fell on the victim’s body, which at that time was near the wood transfer area,” said Inspector Alimuddin.
Colleagues at the location immediately tried to provide assistance by freeing the victim’s body from the pile of wood. However, these efforts did not produce results. Agus Winarno was declared dead at the scene at 08.55 WITA.
Evacuation Takes a Long Time
The process of evacuating the victim took quite a long time because of the large pile of wood that was clamping down on his body. This incident was immediately reported to the Labanan Police Post and the Berau Regional Disaster Management Agency (BPBD) to obtain additional assistance.
“The victim was found lifeless, and the evacuation took quite a long time because of the difficult terrain due to piles of wood,” explained the Police Chief.
Further Research
Currently, the police are still conducting an in-depth investigation regarding the cause of the incident. Several eyewitnesses, including heavy equipment operators and other workers at the location, have been questioned.
“We have also detained the incident location for further investigation,” added Teluk Bayur Police Chief. The accident site on the ship which was used as a place to transfer wood has temporarily stopped activities.

MV Cargo Ship. Kuala Mas Sinks After Collision with a Tanker in Kupang Waters
A cargo ship, MV. Kuala Mas, which sailed from Makassar to Kupang, sank in the waters of Bolok, Kupang, East Nusa Tenggara (NTT), on Saturday (21/12/2024) morning. The ship had an accident after colliding with the Equatorial Maritime tanker as it approached Tenau Harbor.
Videos circulating on social media show the ship’s cargo in the form of containers falling into the sea, while the ship is seen tilting due to the strong impact. A team from Basarnas Kupang immediately went to the location to evacuate the ship’s crew. This incident was reported to have occurred at 02.36 WITA, with 20 crew members successfully evacuated at 08.00 WITA by Polair.
According to Basarnas members, the collision with the tanker caused the Kuala Mas ship to tilt to the right and experience a leak. Although the ship’s captain tried to steer the ship towards shallower waters, these efforts were unsuccessful. As a result, the ship finally sank completely at 08.34 WITA.
Rescue teams worked hard to secure the condition of the crew, and although the ship sank, no casualties were reported in this incident. The evacuation process was carried out smoothly, thanks to fast coordination between the maritime police, Basarnas and related teams.
