From the collision of a Black Hawk helicopter with an American Airlines plane in Washington which claimed 67 lives to the Brimob bus accident on the Pandaan-Malang toll road which killed two people, this series of heartbreaking incidents reminds us of the importance of safety in transportation. Not only in the air and on the highway, major accidents also occurred on other land routes, including a truck collision in West Bandung and the crash of a medical jet in Philadelphia. This article summarizes seven recent incidents that rocked the world, as well as a review from a risk management and insurance perspective.
Tragedy on the Pandaan-Malang Toll Road, Brimob Bus Hits Roadblock. Two People Killed, Dozens Injured
A single traffic accident occurred on the KM 72-73 section of the Pandaan-Malang toll road, precisely in the Purwodadi area, East Java, on Saturday (1/2). This incident involved a bus belonging to the National Police Mobile Brigade carrying a group of class 12 students at SMAN 1 Porong, Sidoarjo, who were on their way to Malang for a yearbook photo session.
The bus had an accident when it hit a road divider (guard rail) and suffered serious damage. As a result of this incident, two people died, namely the bus driver, Khoirul (60), and a class 12 student named Naviri Arimbi Maharani (18).
Chronology of Events
Based on reports from the police, the accident occurred at around 11.30 WIB. The bus carrying 31 students plans to go to the Kayutangan area and Malang City Square for a photo session. However, when crossing the Purwodadi Toll Exit, the bus suddenly lost control and hit a direction pole. The impact was so hard that the front of the bus was crushed, and the driver was pinned inside the vehicle.
Head of PJR Ditlantas Polda East Java, AKBP Hendrix Kusuma Wardhana, said that his party received a report from Jasa Marga and immediately went to the location of the incident. Initial estimates indicate that this accident was caused by the driver’s lack of caution.
Injured Victims and Medical Treatment
Apart from the two deaths, dozens of passengers suffered injuries, both light and serious. The victims were immediately rushed to several hospitals, including Dr. Saiful Anwar (RSSA) Malang, Lawang Medika Hospital, Lawang Regional Hospital, and Prima Husada Hospital.
At Lawang Medika Hospital, there were 16 victims being treated, with three of them having to be referred because their condition was getting worse. Some victims suffered minor injuries, but others suffered serious injuries such as broken bones and head injuries that required intensive medical treatment.
The Head of the East Java Education Service, Aries Agung Paewai, confirmed that the costs of treating all students who were victims of accidents would be covered through BPJS Health. Apart from that, the Pasuruan Police have also prepared a trauma healing program for survivors to help them recover psychologically after the incident.
Warning and Regret from the School
The Principal of SMAN 1 Porong, Ropinggi, revealed that his party had actually banned the activity of taking yearbook photos in Malang. However, the group still left using the Brimob bus. This tragedy is certainly a heavy blow for the school and the victim’s family.
Currently, the police are still carrying out further investigations to find out the exact cause of the accident. This incident is an important reminder of the importance of travel safety, especially for groups of students who use large vehicles on toll roads.

Black Hawk helicopter crashes into American Airlines plane in Washington, killing 67 people
A tragic air accident occurred near Ronald Reagan Washington National Airport on Wednesday (29/1/2025) evening when a Black Hawk military helicopter collided with an American Airlines CRJ700 commercial aircraft. This incident claimed the lives of a total of 67 people and left deep sorrow for the victims’ families and the aviation world.
Accident Moments
Based on preliminary data from the United States National Transportation Safety Board (NTSB), the American Airlines CRJ700 was at an altitude of approximately 325 feet (91 meters) when the collision occurred. Meanwhile, the Black Hawk helicopter reportedly flew above the maximum height limit of 200 feet (61 meters) for the route.
Radar data from the control tower also indicated that the helicopter was at an altitude of 200 feet at the time of the incident. However, the differences in the data are still under investigation.
“That’s our job, to find out the cause,” said NTSB board member Todd Inman, when asked about the discrepancy in the numbers in the initial report.
Before the collision, air traffic controllers were said to have warned the helicopter about the presence of the CRJ700 aircraft about two minutes earlier. In addition, the aircraft’s cockpit voice recorder captured the verbal reactions of the flight crew shortly before the incident occurred.
The automatic transmission system on the plane also gave a warning of “traffic, traffic, traffic” before the sound of the impact was heard in the black box recording, marking the moment of the collision.
Night Vision Goggles Factors Still Under Investigation
The investigation team is also looking into whether the use of night vision goggles played a role in the accident. However, there has been no confirmation whether the device was used by the helicopter crew at the time of the incident.
“We are still looking into whether night vision goggles were used and how they were set. Further investigation will help determine if this was a factor in the crash,” explained Inman.
Victim Identity and Evacuation Efforts
The US Army has identified the three soldiers killed in the Black Hawk helicopter. They are:
- Captain Rebecca Lobach from Durham, North Carolina, who has served since 2019 with the 12th Aviation Battalion, Fort Belvoir, Virginia.
- Staff Sergeant Ryan Austin O’Hara (28)
- Chief Petty Officer 2 Andrew Loyd Eaves (39)
Meanwhile, the names of the 60 passengers and four crew members of the CRJ700 aircraft who died have not been officially announced. However, some of the victims have been identified by their families and social media.
As of Sunday (2/2/2025), the rescue team had recovered 42 bodies from the scene. The process of evacuating plane debris from the Potomac River is also continuing as part of the investigation and search for victims who are still missing.
This tragedy was one of the deadliest air accidents in the United States in recent years and highlighted the importance of aviation safety, for both commercial and military aircraft.

Severe Fire Hits 3 Factories in the Margaasih Industrial Area, Bandung
A large fire hit three factories in the Kampung Sadang industrial area, Rahayu Village, Margaasih District, Bandung Regency, on Thursday (30/1). Thick plumes of black smoke were seen rising high from the fire site, attracting the attention of local residents and making extinguishing efforts difficult.
The Bandung Regency Fire Department (Disdamkar) received a report of the incident at 16.55 WIB. Due to worsening conditions, Disdamkar determined the status “Code Red” and deployed 12 fire engines from various posts in Bandung Regency, with additional assistance from Bandung City and Cimahi City.
Three factories destroyed by fire
This fire destroyed three factories, namely PT Lakistar (accessory manufacturer), PT Aneka Lem (glue manufacturer), and PT Abadi Prima (fabric manufacturer). The size of the fire was caused by the large amount of flammable materials such as cloth, thread, plastic, thinner and glue which accelerated the spread of the fire throughout the factory area.
Efforts to extinguish it were very difficult because the fire continued to grow, made worse by strong winds blowing around the scene. Firefighters worked hard to prevent the spread of the fire from spreading to other buildings.
Disdamkar Enforces ‘Code Red’
When they arrived at the location, firefighters found the fire had spread widely and was difficult to control. Seeing this situation, the Bandung Regency Damkar Service determined the status ‘Code Red’, which means a large scale fire and requires a quick response and additional firefighting units from various areas.
As much 15 fire trucks finally deployed to handle this fire, including assistance from the City of Cimahi and the City of Bandung. Extinguishing is done by spraying water at the main fire point so that it does not spread further. Unfortunately, strong wind conditions and a lack of water supply made extinguishing it even more difficult.
Challenges in the Extinguishing Process
The extinguishing process was full of challenges. Flammable factory materials such as thinner and glue make fires difficult to control with just water. In addition, the absence of functioning hydrants around industrial areas means officers have to look for water sources from other locations, slowing down the rate of blackouts.
“We experienced problems because the hydrant was not working. “The team had to look for a water source in a location quite far away,” said Head of the Bandung Regency Damkar Service, Iman Irianto.
Apart from that, the strong wind also made the situation worse. The gust of wind caused the fire to spread more quickly, so firefighters had to work extra hard to prevent the fire from spreading to other areas.
Fire Extinguished After 13 Hours
After almost 13 hours struggling to fight the flames, the fire team finally succeeded in extinguishing the fire on Friday (31/1) at 05.36 WIB. Although no fatalities were reported, several firefighters suffered minor injuries due to being hit by shards of glass and shards. One of the officers also suffered minor burns due to the extreme heat at the scene.
After the fire was extinguished, officers carried out a cooling process to ensure that there were no hot spots that were still burning and could potentially cause another fire. Meanwhile, the police began investigating to find out the exact cause of the fire.
The cause of the fire is still being investigated
Until now, the cause of the fire is still under investigation by the police. Initial guesses point to the possibility electrical short circuit or chemical reactions from inside the factory as the main trigger for fires. However, the official statement is still waiting for the results of further investigations.
This incident is a reminder of the importance of better fire protection systems in industrial environments. Malfunctioning of hydrants around the factory is one of the factors that prolongs the duration of the fire and hampers extinguishing efforts. It is hoped that further evaluation can prevent similar incidents from recurring in the future.
“When officers arrived, the fire had already spread, so further investigations will be carried out by the police,” concluded Irianto.
This fire incident is one of the largest in recent times in Bandung Regency. It is hoped that preventive measures and thorough evaluation can be carried out to improve industrial safety and prevent similar incidents in the future.

Barge Loaded with Coal Sinks in Tembesi River, Suspected of Polluting Batanghari River Flow
A coal-laden barge reportedly sank at the crossing of the Tembesi River on Saturday, February 1 2025. This incident has become a hot topic of conversation among the local community, especially regarding its potential impact on the environment and water quality in the area.
Jambi Province environmental activist, Jhon Herman, revealed that the sunken barge was Ponton APN 6, which is being pulled by tugboat Devalin. According to him, this incident raises serious concerns regarding the possibility of pollution of the Batanghari River due to the content of dangerous substances in coal.
“With this incident, it is very likely that the Batanghari River flow has been polluted by liquid magma from coal mines, which poses a risk to human health,” Jhon Herman said to the media.
Considering that the Batanghari River is the main source of drinking water for the people of Jambi, he asked the authorities to take immediate action. He insisted Sumatran River Basin Center (BWSS), Jambi Regency Environmental Service (DLH)., And DLH Jambi Province to immediately intervene in response to this incident.
“Pollution in the Batanghari River could be a major threat to people’s health and lives. “Most of the drinking water supply managed by PDAM comes from this river flow,” he stressed.
Until now, there has been no official statement from the relevant parties regarding the exact cause of the sinking of the barge or steps to deal with its impact on the environment. The public and environmental activists continue to highlight this incident, hoping that there will be quick action to prevent wider pollution.
