A series of major accidents has rocked Indonesia in recent days, ranging from fatal toll road accidents, car fires, factories and industrial warehouses destroyed by fire, to light aircraft crashes and oil and gas rig collapses. These incidents have not only caused billions of rupiah in material losses but also reiterated the vulnerability of the transportation, industry, manufacturing, aviation, and energy sectors to operational risks. In this report, we summarize seven significant incidents, along with risk analyses and recommendations for relevant insurance coverage to anticipate similar impacts in the future.
A truck collided with three vehicles on the Semarang toll road, killing the Alphard driver and the conductor.
A multiple accident occurred on the ABC Toll Road in Semarang City, KM 432C towards Tembalang-Gayamsari, on Saturday (11/22/2025). The incident involved four vehicles and killed two people. The incident began when a tronton truck loaded with zinc lost control and hit a tanker truck in front of it. The vehicle then changed lanes and hit a Toyota Alphard and a Daihatsu Terios. The Alphard driver and the tronton truck conductor died at the scene, while several other victims suffered injuries.
Loads of zinc were scattered across the road, causing traffic jams leading to downtown Semarang. Toll road officers and volunteers evacuated vehicles and cleared the debris, while the Semarang Police Traffic Unit conducted a crime scene investigation and further investigation.

A black Nissan X-Trail minibus with license plate D was burned to the ground at the Dustira-Sriwijaya Underpass in Cimahi City on Friday (November 21, 2025). The fire, which occurred at around 1:40 p.m. WIB, caused a long traffic jam from Baros to Cimahi Square.
The Cimahi Fire Department received a report at 2:00 PM WIB and arrived six minutes later. According to Fire Department Squad Commander Indrahadi, the vehicle owner had just purchased the car from the Kopo area and was taking it to a repair shop in Cimahi.
During the trip, the engine suddenly stalled and smoke began to emit. The owner thought the car had overheated, but sparks appeared from the hood. When the hood was opened, the flames grew, causing the owner to panic and call for help from other road users.
Firefighters managed to extinguish the fire in approximately 30 minutes. There were no fatalities; the two people in the car survived. Material losses are estimated at Rp 100 million. During the extinguishing process, traffic from Baros was temporarily diverted, causing a 2-kilometer-long traffic jam.

A major fire broke out at PT TAK Textiles Indonesia, located on Jalan Hajar Mekar, North Cikarang, Bekasi Regency, early Tuesday morning (November 25, 2025). The fire, which broke out around 2:10 a.m. Western Indonesian Time (WIB), quickly spread and engulfed the production area containing highly flammable fabrics, causing losses estimated at billions of rupiah.
Initial police information indicates the fire is suspected to have originated from an electrical short circuit in one of the warehouses. The dense construction of the building and the large amount of textile materials allowed the fire to spread quickly to other areas.
The Bekasi Regency Fire Department deployed a total of 11 fire trucks, seven from Bekasi Regency and four to five from the Jababeka area, to extinguish the fire. Crews faced challenges due to the tight structure, limited access, and the fabric material, which required longer cooling time.
All employees were successfully evacuated with no reported casualties. As of this writing, extinguishing and cooling efforts are still ongoing. Further investigation into the cause of the fire will be conducted by the Bekasi Metro Police and the North Cikarang Police.

The fire broke out shortly after workers had returned home and the building was closed. Residents who saw the flames reported the incident, and firefighters were immediately deployed.
The highly flammable plastic material caused the fire to spread rapidly, taking nearly 10 hours to extinguish. The fire was finally extinguished at 3:00 a.m. Western Indonesian Time.
Although there were no casualties, material losses are estimated at Rp 1.5 billion. Initial suspicions point to an electrical short circuit. Cessna Plane Crashes in Karawang Due to Aircraft Problems
A Cessna aircraft belonging to BRO Skydive Indonesia with registration PK-WMP was reported to have crashed in Kertawaluya Village, Tirtamulya District, Karawang Regency, West Java, on Friday (21/11/2025) at around 15.00 WIB.
According to statements from residents submitted to the Karawang Police, the plane appeared to experience problems while still in the air before crashing in an open area. The plane had taken off from Tangerang bound for Cirebon and was carrying five crew members.
The Karawang Police reported that all crew members were safe and evacuated to Trimulya Community Health Center. Karawang Regent Aep Syaepuloh immediately inspected the scene and ensured that the crew members were receiving optimal medical treatment and examinations. There were no serious injuries in the incident.

A tragic incident occurred on the AU17 HO-Duri rig owned by PT Artindo Utama on Monday afternoon (November 24, 2025), killing one derrickman and injuring two other workers, who suffered broken hips. The accident occurred around 11:00 a.m. WIB (Western Indonesian Time) while the rig was operating at high altitude.
According to the initial investigation, the accident occurred during the Running Down power swivel process after the Drilling Out Cement work. When the drillpipe was lifted, the rig mast suddenly bent and fell towards the catwalk, hitting derrickman A (43) who was on the monkey board, and was hit by the counterweight of the parked mobile crane.
Pertamina Hulu Rokan (PHR) and the police are still investigating the cause of the rig collapse. All victims were immediately evacuated to medical facilities, while the operation site was secured. This incident has heightened public concern about safety standards in the high-risk oil and gas drilling industry.

The lack of firefighting equipment caused the fire to get out of control and spread quickly to eight other shophouses. Karangasem Fire Department personnel had difficulty reaching the location, so assistance from the Klungkung Fire Department arrived approximately 40 minutes later. Each vendor is estimated to have suffered losses of IDR 35–40 million, not including damage to building facilities. The cause is still being investigated, but initial suspicions point to a short circuit or incense fire left over from prayers.
