Liga Asuransi – Tragedies and unexpected incidents continue to be a concern in various parts of the world. From the fire that hit a garment factory in Grobogan to a work accident in Dumai, these events remind us of the importance of vigilance, safety and risk management. This article will review a series of shocking events that have recently occurred, providing an in-depth overview of the causes, impacts, and preventive steps that can be taken to prevent similar incidents in the future. Let’s explore further the details of each of these events.
Successive accidents on the Cipularang toll road, allegedly caused by trucks not being able to climb uphill
On Sunday, January 5 2025, at around 09.11 WIB, a series of accidents occurred at KM 97+200 of the Cipularang toll road section towards Bandung. The accident involving around five vehicles is thought to have been caused by a truck that was unable to climb the route.
According to Agni Mayvinna, Senior Manager Representative Office 3 Jasamarga Metropolitan Tollroad Regional Division, the incident started when a truck passing by the accident location failed to negotiate the incline on the Cipularang toll road section towards Bandung. As a result, the truck reversed, causing a collision with another vehicle behind it.
Officers in the field explained that the contour of the road at KM 97+200 was known to be uphill, which was the main factor causing the incident. In these conditions, heavy vehicles such as trucks often have difficulty maintaining speed when traversing hills, which can lead to accidents if the vehicle is not strong enough to climb the hill.
This accident involved more than five vehicles colliding with each other, causing serious traffic jams on the route. The authorities immediately handled the incident location and carried out evacuations and clean-up to reduce the impact of traffic jams.
This incident is a reminder for motorists, especially truck drivers, to always check the capabilities of their vehicles before crossing uphill roads, as well as pay attention to weather and road conditions that can affect their journey.

Accidents in a Series on the Semarang-Solo Toll Road, Bus Hits Truck Loaded with Hydrogen Peroxide
On Friday, January 3 2025, at around 07.20 WIB, a series of accidents occurred on the Semarang-Solo toll road, precisely in Pringapus District, Semarang Regency. The incident involved a bus, an Avanza car and a truck carrying a load of hydrogen peroxide, a textile bleaching chemical.
Semarang Police Traffic Unit Head AKP Lingga Ramadhani explained that the accident started when the bus with plate number DK 7169 GH, driven by Dian Fajar (22) from Tulungagung, was traveling quite fast on the toll road. At that time, the bus tried to overtake an Avanza car with plate number BN 1617 QK driven by Ali Akbar (48) from Bangka Regency. The bus then sideswiped the Avanza and swerved to the left, hitting a truck with registration number W 8930 CA which was transporting hydrogen peroxide.
Even though the video circulating shows smoke rising at the accident site, AKP Lingga Ramadhani confirmed that the truck’s cargo was not dangerous. “The truck was carrying hydrogen peroxide, a textile bleach. This material is not dangerous and dissolves easily in water,” he explained.
As a result of the accident, the truck driver, Andi Setiawan (26), suffered minor injuries to his forehead. Luckily, the chemicals carried by the truck did not cause any further harm. Officers immediately cleaned the incident area with water to ensure no dangerous residue remained.
Dian Saputra, Public Relations Manager of PT Transmarga Central Java (TMJ), also emphasized that the chemicals carried by the truck were not dangerous. “This material is a disinfectant or textile bleach, which is safe and easily dissolved in water,” he said.
The police are currently investigating the three drivers involved in this accident. Although the accident caused congestion on the motorway, the situation is under control and there is no further risk associated with the truck’s load.

Bicycle Spare Parts Factory, PT Roda Pasifik Mandiri Fire in Semarang, Allegedly Started from Sparks on the Roof of the Warehouse
On Thursday, November 21 2024, at around 16.16 WIB, a large fire broke out at the PT Roda Pasifik Mandiri bicycle and electric bicycle spare parts factory in Semarang City, Central Java. The fire, which is thought to have originated from the roof of the warehouse building, sparked panic among employees, who ran to save themselves when thick smoke began to soar.
Semarang Deputy Chief of Police, AKBP Wiwit Ari Wibisono, confirmed that the police were still carrying out investigations to find out the exact cause of the fire. “Still under investigation, we don’t know the exact source of the fire, but based on witness statements, initially there were sparks on the roof wall. Whether it was a short circuit or not, we will investigate further after the fire is extinguished,” said Wiwit at the scene.
The fire, which involved around 15-20 fire engines, was only brought under control at around 19.40 WIB. Even though the fire was quite large, fortunately there were no casualties in this incident. However, a firefighter suffered minor injuries as a result of being hit by a light fire extinguisher (APAR).
One of the factory employees, Misbah, who works in the raw materials storage warehouse, revealed that he first heard the sound of an explosion at around 16.30 WIB, before realizing there was a fire. “The fire started out small, but because the materials in the warehouse were flammable, the fire quickly grew. This is the first fire after 15 years of this factory’s existence,” said Misbah, who managed to escape the fire.
Authorities are currently still working to extinguish the fire and investigate further the cause of this disturbing fire. This fire is a reminder of the importance of monitoring electrical installations and safety in areas where flammable materials are stored.

Cargo Ship Loaded with Cement Sinks, One Crew Dies and One Still Missing
A cement-laden cargo ship, the Jerlyn Kathness, sank in central Philippine waters on Monday, December 30, 2024, in the San Bernardino Strait near Samar Island. This incident resulted in one crew member being killed, while another crew member is still missing. Of the total 15 crew members, 12 were rescued, and another crew member was found swimming towards San Juan Island on Tuesday morning (31/12/2024).
The Philippine Coast Guard, which led the search and rescue operation, confirmed that the ship sank due to large waves. Very rough sea conditions and strong currents in the San Bernardino Strait further worsened the situation. The ship carrying cement from Cebu Island to Samar sank in waters known to be dangerous.
Local fishermen found the body of one of the crew near Biri Island, while the search for the missing crew continues. The Jerlyn Kathness, a 199 tonne vessel, was built in 1993 and was reported to be in seaworthy condition before the incident occurred. Authorities are now investigating the exact cause of the ship’s sinking.
The search operation involved coast guard vessels and local fishermen. The surviving crew have been evacuated to safer places. Meanwhile, the authorities appealed to other ships to be more careful when crossing the San Bernardino Strait and pay attention to shipping safety, especially in areas prone to large waves like this.
